Apply to SJS

If you are a new family, you can fill out an online application packet online thru RenWeb. There is a $100 application fee per child and a $400 enrollment fee per child.

Students entering PreK must be 4 years old on or before September 1st. 
 Students entering Kindergarten must be 5 years old on or before September 1st.

Applying to attend SJS is ongoing throughout the year. New students in grades Kindergarten through seventh grade are given a placement test to ensure that SJS can meet their needs and that they will be successful.

For all students, the following documents must be provided before enrollment is complete:

• Birth Certificate (Official Copy)
• Baptismal Certificate
• Emergency Contact Card
• Immunization Records***
• Diocese of Fort Worth Health Report
• Copy of last report card (Grades 1-8)
• Copy of most recent standardized test scores (Grades 3-8)
• Transcript request form, signed (Grades 1-8)

***Every student enrolled in a Catholic School in the State of Texas shall be immunized against vaccine preventable diseases caused by infectious agents in accordance with the immunization schedule adopted by the Texas Department of State Health Services. A student who fails to present the required evidence shall not be accepted for enrollment. The only exception to the  foregoing requirements is a medical exemption signed by a licensed physician (M.D. or D.O.) authorized to practice in the State of Texas, including the physicians license number. Immunizations are not in conflict with the Catholic Faith. Conscientious objections or waivers, which may be permissible for attendance in public schools, do not qualify as an exemption in Catholic Schools in Texas (Atty. Gen. Op. GA-0420). This policy was adopted by Texas Catholic Conference Education Department, December 2008 and voted on April 3, 2017 to keep as written with no changes by the Bishops of Texas.  

Our dress code does require students in Pre-Kindergarten - 8th grade to wear a school uniform. Uniforms may be purchased at Dennis Uniform, our School ID Number is MI3420. Used uniform sales are hosted twice a year at the school. 

Admission to St. John the Apostle Catholic School or participation in school programs is not denied students on the basis of race, color, ethnic or national origin. Admission shall not be denied because of any disabling condition unless it is clear that the staff, due to inadequate professional training and/or the school's program, will not be able to meet the student's needs.
